Rake in the rewards. You and your eligible spouse can each earn up to $500 in rewards. You can get rewards deposited into your health savings account (HSA) or exchange rewards for e-giftcards to some of your favorite brands. Complete your activities by December 31, 2022.

Get a screening to learn your numbers for BMI, blood pressure, cholesterol and glucose. You'll earn $100 just for completing your screening.

And, if you meet certain biometric targets, you’ll automatically earn $25 per goal:

1. Body mass index (BMI): ≤ 27.5 kg/m2 or an improvement of ≥ 2 points	$25
2. Blood pressure: ≤135/80	$25
3. Total cholesterol: <200 mg/dL	$25
4. Glucose: < 100 mg/dL or A1C of <5.7%	$25

3 ways to complete your biometric screening: an on-site event, doctor’s office or participating Quest lab. To earn your Health Rewards, you must register for your screening through my.questforhealth.com and use code JLL2022. If you experience login issues, please contact Quest Diagnostic’s customer service line at 1-855-623-9355.

If biometric screening is completed with your health care provider, you must print out the Health Provider Screening Form at my.questforhealth.com, have your provider complete it and upload to Quest (directions are on the form).

A Personal Health Action is a goal you decide on to improve your health. You work with a coach to set a goal, like walking twice a week, and then work toward it. And then you get money.

Call myBLUEPRINT4HEALTH at 1-800-996-2057 by October 1, 2022, so you have enough time to complete the program by the December 31, 2022 deadline.

Note: If you are pregnant, you can earn $100 for completing a Personal Health Action within the Maternity Support Program in place of completing a Personal Health Action with a health coach. Call 1-800-996-2057 to enroll in Maternity Support.

You and your covered spouse/domestic partner can earn $100 each for visiting your doctor for an annual preventive exam or prenatal care. Go to myuhc.com or call myBLUEPRINT4HEALTH at 1-800-996-2057 to find a network doctor near you. Your preventive exam will be credited toward your Health Rewards when the claim is processed, and no further action from you is required.

Look for the Tier 1 symbol on myuhc.com. To get the highest level of care at the best value, make sure you’re choosing Tier 1 providers. Look for the Tier 1 blue dot icon next to your doctor’s name on myuhc.com.

Covered at 100%. As part of your recommended care, an annual exam is covered 100% by JLL when you use a UnitedHealthcare network provider.

Here’s a tip: If you're planning to complete your biometric screening with your provider at the same time as your preventive exam, you must print out the health provider screening form at my.questforhealth.com, have your provider complete it and upload it to Quest (directions are on the form). Use code JLL2022 to register on the site.

Call myBLUEPRINT4HEALTH at 1-800-996-2057 to learn about your maternity benefits and join the Maternity Support program. You can complete the Maternity Support program in place of completing a Personal Health Action or Rally Coach program.

Note: Expecting mothers are encouraged to participate in Maternity Support within the first 20 weeks of pregnancy.
